Yup, it's definitely Fred. First case he kept getting in my way. Then the surgeon resident started making snide comments.

I'm amazed at Fred's command of commentary on that fellow's culinary, sanitary, and sexual habits.

I think it comes from actually having been face to face with Fred.
It's the accent. It gets stuck and you come down with a bad case of Fred-in-de-Head.
He does that to me sometimes and I CAN NOT kick the bastard out.
He makes rude comments on everything and it would be very counter-productive to voice those things.
But they can be highly amusing.
